1. This booking is a contract between you the diver(s) and Piranha LLC. We ask you to please only submit a booking for an itinerary you are actually able and willing to join. Please check our terms and conditions before booking. (Point 6: Responsibilities, Point 8: Cancelation Policy)
2. Submit one booking per family or group and mention all members of your party in the bottom part of your booking! To avoid confusion please refrain from filling several bookings for several participants. Please use the same contact data as on initial contact to avoid confusion.
3. Make sure to mention the program and dates you want to join correctly. That enables us to send you correct invoices, e-learning codes and allocate an Instructor and avoid over bookings or last minute changes.
1. Certified divers will not be required to fill out a medical form or get doctor’s clearance.
2. For any diving course, you must fill out a medical self declaration. You need to be able to answer “NO” to all questions on the diver medical form to join. If you answer “YES” to any of the questions, you need doctors approval. Use the form below to confirm if doctors approval is needed or not. If not needed, fill in the form on the day you start your course.
3. If you are 60+ years old: We will require a medical waiver signed by a doctor when joining any diving course. We strongly recommend a doctors approval before joining any diving activity for your own peace of mind.
4. Piranha LLC has a general liability insurance as required by Japanese law, but you the diver are ultimately responsible for your personal dive and travel insurance to cover diving incidents, missed dives and other unwanted situations. We recommend Diveassure, Divers Alert Network (DAN) and/or DAN Trip Insurance and/or Aquamed for European Divers. Japan is a country where english is not always spoken or understood, being able to speak to someone in your language when help is needed is a good thing. Some providers even offer coverage for a single day of diving, or more.
